2009 Seminole Chief Registration Dates
***All Registrations at SYAA Complex***
 
+++ Play It Again Sports will offer packages for pants pads, girdles and chinstraps.  Detailed information available at registration+++
Conditioning Camp June 22nd through 26th.  Conditioning Camp Registration Fee is $40.
 
At Registration:
 
1. Must have parent or legal guardian present with valid Driver's License or State Issued I.D. Card. If any person other than the parent listed on the birth certificate registers the child, legal documents showing the person has custody must be presented at registration.
 
2. Birth Certificate: must be legible copy of child's birth certificate or hospital record with the seal containing the names of the parents. A notification of birth record or a birth card is not acceptable.
 
3. Two (2) COLOR school/passport type pictures of the child 2" x 2" in size. The pictures must be recent and look like the child. The child's face must be clearly visible (head shot). No hats, hair or other objects that impair facial features. It is preferred that the two pictures are identical.

***ATTENTION PARENTS***

Marc Estrada Photography will be back this season taking action photos and creating a Montage in a variety of sizes, Vinyl Cutouts, 20 page Flip book and more. If you want to sign up to order photos you must sign up early at his blue tent which will be set up at every home and away game. He suggests you sign up as early as possible. He will be set up at the jamboree for you to sign up also. Sign up fee is $20 per child which is not refundable but will be deducted from your final order.
